Biologics Home Care Nurse (Full Time)
Monday to Friday (no week end work)

Role Summary
To deliver a Patient Support Programme for patients who have been initiated onto a biologics treatment.
The purpose of this role is to improve patient outcomes and deliver a high standard of care.
Key Aims and Responsibilities
- To work effectively and flexibly to deliver the Homecare training to new and existing patients.
- To provide an omni\-channel (telephone and virtual) based service which can include regular proactive outbound support and reactive inbound support to drive patient education, concordance and compliance.
- To take part in the rota (Matrix) ensuring all patients are contacted and offered the appropriate nursing support in a timely way
- To deliver the service to enrolled patients both face to face, through homecare visits, and via telephone and virtual calls.
- To deliver, or administer or train patients on any diagnostic tests that may be required as the programme develops.
- Provide advice and education on treatment and lifestyle recommendations.
- Ensure that patients are treated with dignity, equality and respect
- Record all patient data using the CRM system and ensure all administration is completed in a timely and accurate manner.
- Working professionally and effectively to build relationships with all stakeholders (clients, NHS consultants, Pharmacists, Nurses, other MDTs and internal staff).
- Current NMC Registration with specific experience in biologics administration (min Band 6\).
- Biologics experience is essential. Experience of working in rheumatology, gastroenterology and/or dermatology preferred.
- Current U.K. full driving licence with no more than 6 points and an ability and willingness to travel to Trusts and patients’ homes within an acceptable geography.
- Excellent interpersonal skills and communication skills
- High levels of confidence, initiative \& proactivity
- IT literate with experience of MS office specifically MS Teams, Excel, general experience of using computerised patient management systems.
