<p><strong>Banding:</strong> Band 7<br><strong>Location:</strong> Hammersmith Site<br><strong>Start Date:</strong> 15/01/26<br><strong>Duration:</strong> 3–4 months<br><strong>Working Hours:</strong> Full‑time, Monday–Friday, 9:00am–5:00pm (7 hours per day)<br><strong>Hourly Rate:</strong> £23-£30</p>
<p><strong>Job Overview</strong></p>
<p>We are currently seeking an experienced Band 7 Clinical Pharmacist to join the team at the Hammersmith site on a full‑time basis. This is a fantastic opportunity for a dynamic pharmacist with critical care or cardiology experience to provide high‑quality clinical pharmacy services across the hospital.</p>
<p>The successful candidate will deliver patient‑centred pharmaceutical care, work closely with multidisciplinary teams, and contribute to safe, effective medicines optimisation.</p>
<p><strong>Key Responsibilities</strong></p>
<ul>
<li>Provide day‑to‑day clinical pharmacy services across allocated wards, with a focus on critical care and/or cardiology.</li>
<li>Deliver medicines reconciliation, clinical screening and optimisation to improve patient outcomes.</li>
<li>Support safe and effective prescribing through clinical advice and therapeutic recommendations.</li>
<li>Participate in multidisciplinary ward rounds and liaise with medical and nursing staff.</li>
<li>Ensure compliance with local and national guidelines for medicines management.</li>
<li>Contribute to discharge planning, ensuring accurate and timely provision of medicines.</li>
<li>Promote antimicrobial stewardship and safe prescribing practices.</li>
<li>Support junior staff and participate in departmental learning and development activities.</li>
</ul>
<p><strong>Skills & Experience Required</strong></p>
<ul>
<li>Registered Pharmacist with the <strong>GPhC</strong>.</li>
<li><strong>Essential:</strong> Experience in <strong>critical care or cardiology</strong>.</li>
<li>Previous acute hospital clinical experience desirable.</li>
<li>Strong communication and multidisciplinary teamwork skills.</li>
<li>Ability to work independently and manage clinical priorities effectively.</li>
