ANP/Nurse Practitioner – Home Visiting & Frailty

We are looking for an experienced and motivated Advanced Nurse Practitioner/Nurse Practitioner to join our Home Visiting Team.

The successful candidate will provide high\-quality care to frail and housebound patients within the community, undertaking home visits, clinical assessments, treatment, and care planning.

You will work closely with GPs, care homes, community teams, and the wider multidisciplinary team to support proactive patient care and help reduce avoidable hospital admissions.

About You

Essential

• Registered Nurse with current NMC registration


• Confident working autonomously and as part of a multidisciplinary team


• Full UK driving licence and access to a vehicle

• Advanced Clinical Assessment qualification or equivalent experience


• Independent prescriber


• Experience in primary care, community, urgent care, or frailty services
