Ward Manager - Ward C9
As a new way of working in the Medicine and Emergency Care, Care Group we have designated Ward C9 as an area to cohort medically optimised for discharge. patients waiting for:
Pathway 1, Home First discharge
ii) Pathway 1, Reablement, Package of Care, Package of Care restarts, Early Supported Discharge (ESD)
iii) Pathway 2 patients for residential care homes from Blackburn with Darwen only.
iv) Waiting for the delivery of specialist equipment
v) Lancashire County Council patients with a confirmed discharge date to a residential care home
This model means the ward is nurse\-led, with some therapist support if required, and medical cover only an as\-required in\-reach support for patients who may relapse. The proposal is the patients are managed to their discharge by the nurses, therapists, and discharge support teams, depending on the patients’ particular needs, with a clear focus on criteria led discharge.
You will be responsible for the leadership and development of the ward nursing team.
It will be crucial as the ward manager that you will actively seek the support and positive engagement of key stakeholders in the discharge process: including but not limited to Discharge Matron, Discharge Lounge, Clinical Site Management, Intensive Home Support, Intermediate Care Teams, Therapists, Pharmacy, Integrated Discharge Service (IDS), Ward Managers and Ward Coordinators.
It is essential that you you will have the leadership and effective communication skills to ensure that suitable patients as per the criterion in the Standard Operation Procedure is followed with close working with the IDS Team and MEC Bed manager/Clinical Flow Team \& Discharge Matron to ensure the best use is made of the Ward.

Ensuring best practice in infection control.
In conjunction with the Matron ensure all quality improvement work
is implemented and sustained.
Ensure an excellent patient journey, safe and effective discharge
planning to achieve a positive patient experience.
