Transitions Social Worker
<div>Liquid Personnel is recruiting a Transitions Social Worker for its client in Bristol to support young adults aged 16 to 25 moving from children to adult social care.</div>
<p><strong>What will your responsibilities be?</strong></p>
<p>You will hold a caseload of adults while working closely with children’s services to ensure smooth transitions. The role requires strong knowledge of key legislation and safeguarding processes, alongside collaborative multi-agency working.</p>
<p><strong>Key Requirements:</strong></p>
<ul>
<li>Qualified Social Worker with Social Work England registration</li>
<li>Strong knowledge of the Care Act, Mental Capacity Act, best interest processes and safeguarding, including s42 enquiries</li>
<li>Experience supporting young people or adults with complex needs</li>
</ul>
<p><strong>Desirable:</strong></p>
<ul>
<li>Experience with DoLS, Court of Protection, CHC, SEND or EHCP</li>
<li>Familiarity with LAS system</li>
</ul>
<p><strong>Working Pattern:</strong></p>
<ul>
<li>Full-time</li>
<li>2 days per week in the office at City Hall (Wednesday plus one other day)</li>
<li>Home working available on other days</li>
<li>First week fully office-based for training and induction</li>
</ul>
