Support Worker
About the Role
We are looking for caring, reliable and compassionate Support Workers to join our healthcare team. You will support adults and children with a range of needs, helping them maintain their independence, dignity and quality of life.
We are a domiciliary care provider offering person\-centred care and support within the community. Our services cover personal care, social inclusion community support and supported living. As a Support Worker, you will work directly with individuals in their own homes and within the community, supporting their independence, wellbeing and day\-to\-day needs.
Key Responsibilities
- Providing person\-centred care and support to individuals in their own homes or supported living environments.
- Supporting with personal care, including washing, dressing, toileting and continence care
- Assisting with meals, medication and daily routines where required
- Supporting individuals with mental health needs, promoting emotional wellbeing and independence
- Providing companionship and encouragement with social activities and community involvement
- Supporting individuals with household tasks and daily living skills
- Following individual care and support plans and risk assessments.
- Working closely with families, healthcare professionals and other members of the care team
- Promoting dignity, choice, privacy and independence at all times
- Reporting any concerns regarding health, wellbeing or safeguarding appropriately
