Service Users: Adults aged 18–65 with autism, ADHD and complex mental health needs
Hours: Full\-time, part\-time and bank shifts available (including days, nights, weekends and bank holidays)
Location: Manchester and Greater Manchester

Priority Healthcare Services provides bespoke supported living for adults aged 18 to 65 who live with autism, ADHD and complex mental health needs. We are a non\-regulated service, which means we focus on flexible, person\-centred support tailored to the individual rather than a one\-size\-fits\-all model. Every person we support has their own goals, preferences and aspirations, and our role is to walk alongside them as they live the life they choose.
We are looking for experienced Support Workers who are genuinely motivated to make a difference — people who bring patience, empathy, energy and resilience to everything they do.
Role Purpose:
As a Support Worker, you will provide high\-quality, person\-centred support to individuals in their own homes and in the wider community. You will help the people we support to live independently, build confidence, develop daily living skills, manage their wellbeing and engage in activities that matter to them. You will be a positive, consistent presence — someone who listens, advocates and empowers.
Key Responsibilities:
- Deliver tailored support in line with each individual’s personalised support plan, positive behaviour support (PBS) plan and risk assessments.
- Build trusting, respectful relationships with the people you support, taking time to understand their communication style, sensory needs and preferences.
- Promote choice, dignity, independence and inclusion in all aspects of daily life.
- Support individuals to set and work towards personal goals, whether that’s learning new skills, accessing the community, education, volunteering or employment.
- Recognise early signs of distress, anxiety or mental health deterioration and respond in a calm, consistent and trauma\-informed way.
- Use de\-escalation techniques and proactive strategies in line with each person’s behaviour support plan.
- Support individuals to attend appointments with mental health professionals, GPs and other healthcare providers, and to follow agreed treatment or therapy plans.
- Promote emotional wellbeing through meaningful activity, routine and positive relationships.
- Help with everyday tasks such as cooking, cleaning, laundry, shopping, budgeting and managing the home.
- Support safe administration of medication in line with company policy and individual MAR charts.
- Encourage and support healthy lifestyle choices, including nutrition, exercise and sleep routines.
- Support individuals to access the community safely — from local shops and leisure activities to holidays and family visits.
- Maintain accurate, timely and professional daily records, incident reports and communication logs.
- Contribute to reviews of support plans, risk assessments and behaviour support plans.
- Work closely with colleagues, family members, social workers, CPNs, psychiatrists and other multi\-disciplinary professionals.
- Report safeguarding concerns, incidents and near\-misses in line with company policy.
- Uphold safeguarding principles at all times, protecting the people we support from abuse, neglect and exploitation.
- Follow all health and safety, infection control and lone working procedures.
- Adhere to Priority Healthcare Services’ policies, procedures and code of conduct.
- Maintain confidentiality in line with GDPR and data protection requirements.
- Minimum 1 year’s paid experience supporting adults with autism, ADHD, learning disabilities or complex mental health needs.
- Sound understanding of person\-centred support and positive behaviour support (PBS).
- Excellent communication skills, including the ability to adapt communication for individuals with autism or sensory needs.
- Calm, resilient and emotionally mature — able to manage challenging situations professionally.
- Strong written English for accurate record keeping.
- Willingness to work flexibly, including evenings, weekends, sleep\-ins and bank holidays.
- Right to work in the UK.
- Right to drive in the UK.
- Willingness to undergo an Enhanced DBS checks.
- Level 2 or 3 Diploma in Health and Social Care/NVQ (or working towards).
- PBS Training, Breakaway Techniques.
- Experience supporting individuals under the Mental Health Act, CTOs or Section 117 aftercare.
- Knowledge of the Mental Capacity Act 2005, DoLS and Safeguarding Adults Procedures.
- Full UK driving licence and access to own vehicle.
