An exciting opportunity has arisen for a secondment post as a Support, Time and Recovery (STR) Worker within the East Riding Older People’s Community Mental Health Team (CMHT). The role will be based at either Willerby or Goole and will cover the Goole, Haltemprice, and Beverley areas.

Experience of working within a CMHT would be advantageous, as would previous roles within Mental Health Services. We are looking for someone who demonstrates a flexible, innovative, and person\-centred approach to practice.

You will be responsible for providing interventions to service users (Adults mostly over the age of 65\) and carers living in the community or community settings such as residential homes, experiencing or recovering from mental health difficulties.

You will work within a multi\-professional team to support and assist care coordinators in assessing, planning, implementing, and evaluating care plans for patients on your caseload.

You will be expected to establish therapeutic relationships that support recovery goals, enabling and empowering individuals to access a range of local community resources and services that promote their recovery.

To provide support to service users with mental health needs by promoting a Recovery Model of Care by providing support, directly and flexibly.

To work directly with service users, on an individual and/or group basis.

To assist and support care coordinators and lead professionals to provide treatment and support.

To work across boundaries of care focusing on the specific needs of service users in accordance with their care plans, working in partnership with the individual and their care team.

The post holder will promote assist with basic practical tasks, promote independence, signpost into local organisations in an attempt to develop and encourage links within the local community.
