Support Practitioner

What the job will involve
At Cornerstone, the people we support are at the heart of everything we do. You'll work alongside individuals to help them achieve their personal goals, make choices and live as independently as possible.
Every person is different, with their own interests, routines and ambitions. Your role will be to provide personalised support that helps people to:
Manage their own homes and develop their independence
Access personal care and support their health and wellbeing

Get out and about in their local community
Work towards their individual goals and aspirations

Shift patterns
Our community support team provide tailored support typically between the hours of 9am and 6pm. You may be supporting more than one individual in a day. Weekends and public holidays would also form part of your rota on a rotational basis.
Successful candidates will be required to join the Protection of Vulnerable Groups Scheme and be committed to working towards an SVQ in Social Services and Healthcare with the support from our Training Academy.
Please note \- If you have spent more than 12 months out with the UK (excluding France, Germany. Greece, Hungary, Ireland, Italy, Lithuania, Netherlands, Poland, Portugal, Romania or Spain) within the last 10 years while over the age of 18, you will be asked to provide a criminal record certificate from the relevant government/police authority as part of our pre\-employment checks.
If you’re successful in joining our team, we will need you to register with the SSSC within 3 months of starting your employment with us.
