Social Worker - Hospital Discharge Team
<p><span data-olk-copy-source="MessageBody">Liquid Personnel is seeking an experienced Social Worker to join its clients Hospital Discharge Team, supporting citizens across discharge pathways. </span></p>
<p><strong>What will your responsibilities be?</strong></p>
<ul>
<li>Assess and support citizens on discharge from hospital across three pathways</li>
<li>Complete post-discharge assessments and reviews</li>
<li>Undertake Care Act Assessments (CAA) and support planning</li>
<li>Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work</li>
<li>Lead on safeguarding enquiries and interventions</li>
<li>Provide duty cover as required</li>
<li>Attend Quality Assurance panels for funding provisions</li>
<li>Support Discharge to Assess (D2A) pathways</li>
</ul>
<p><strong>Requirements:</strong></p>
<ul>
<li>Proven experience within hospital discharge settings</li>
<li>Strong working knowledge of Care Act, MCA, safeguarding, and discharge processes</li>
<li>Ability to manage a fast-paced caseload and work under pressure</li>
<li>Excellent assessment and decision-making skills</li>
<li>Available to start immediately</li>
</ul>
<p><strong>Qualifications & Experience:</strong></p>
<p>To be successful in this role you must have, </p>
<ul>
<li>Social Work England registration</li>
<li>Eligible to work in the UK</li>
<li>Hold a full UK driving licence</li>
<li>Post-qualified experience</li>
<li>Degree level or equivalent in Social Work</li>
</ul>
