Our client, Leicestershire County Council, is seeking an agency social worker for its Adult Mental Health team.
This is a temporary ongoing role paid @ £34 per hour.
It covers the Loughborough/Charnwood area of Leicestershire, although the office base is County Hall.
Details of the role:
- Charnwood Mental Health and Reablement team
- Manage a complex caseload of adults with mental health needs, undertaking Care Act assessments, reviews, risk assessments, and support planning.
- Act as Care Coordinator where appropriate, working closely with NHS Mental Health Services, Community Mental Health Teams, carers, and partner agencies to deliver person-centred outcomes.
- Undertake safeguarding enquiries, Mental Capacity Act assessments, best-interest decision making, and complex risk management to ensure people are supported safely.
- Promote recovery, independence, social inclusion, and access to community resources, while supporting effective hospital discharge and preventing unnecessary admissions.
- Prepare high-quality assessments, reports, and case records, and contribute to multidisciplinary meetings, reflective practice, and team duty functions.
- Hybrid role. Attendance at the office is required for group supervision, reflective practice, team meetings and office-based duty on a rota basis when required. Typic office attendance is 1-2 days per week.
Requirements of the role:
- Qualified Social Worker
- Fully Social Work England registered
- Must have significant post-qualified experience in adult social care
- Must have experience working within Adult Mental Health services or a Community Mental Health team.
- Strong working knowledge and practical experience of the Care Act, Mental Capacity Act, safeguarding adults, risk management, and complex case work.
