We're seeking a committed and skilled Social Worker to join the Adult Community Team in Oxfordshire. This is an excellent opportunity to make a real impact within the community-supporting adults to live independently, safely, and with the right networks around them.
Working within Adult Social Care, you'll use a strength-based approach to carry out assessments, coordinate support, and collaborate closely with health partners, families, carers, and community organisations. You'll play a vital role in promoting wellbeing, resilience, and positive outcomes for the people you work with.
Your new role
As part of the Community Team, you will:Complete strength-based assessments, reviews, and interventions.Support individuals under the Care Act 2014, MCA 2005, and relevant legislation.Develop clear, outcome-focused support plans tailored to individual needs.Work with multidisciplinary teams, community partners, and families to achieve the best outcomes.Provide advice on Assistive Technology, equipment, reablement services, and housing adaptations.Manage a varied caseload, including duty responsibilities and community visits.Maintain accurate, defensible case records and contribute to service development.
What you'll need to succeed
You're a confident, resilient social worker who is passionate about delivering high-quality social work and empowering individuals.Essential:Recognised Social Work qualification & Social Work England registrationStrong knowledge of Adult Social Care legislation and safeguardingAbility to work both independently and within a multidisciplinary teamExcellent communication, assessment, and report-writing skillsCommitment to professional development and reflective practice
