Job Details
An exciting opportunity has arisen to join the Outreach Team. This is a dynamic and rewarding role, supporting adults with physical and/or learning disabilities, complex mental health issues including Autistic Spectrum Disorders as well as adults who have offending histories both in their own homes and to help them with inclusive integration and participation in their communities.
This is far from a typical role. You will be part of an Outreach model team providing a wide range of supports to adults who may have physical and/or learning disabilities, challenging behaviour and complex mental health issues including Autistic Spectrum Disorders. You will also support adults who have offending histories. Support is provided both in their own homes and to help them with inclusive integration and participation in their communities. You will promote social inclusion by working with people to maximise their personal independence, health and wellbeing and to access opportunities in their local communities.
The job involves providing person centred care and will be focused on promoting independence and skills development and may require specific skills in behaviour management and it is essential you understand the impact of learning disabilities on supported individuals. Duties may include all aspects of personal care, and you must have the physical ability to undertake all Moving and Handling manoeuvres and tasks including the pushing and pulling of wheelchairs.
This role will include working collaboratively within multi\-disciplinary teams to which you will have critical input in relation to report writing, person centred plan development and key team responsibilities relating to case management of specific individuals as part of a wider support team. You will keep accurate records of outcomes focused support planning and review for the people whom you support. You will ensure changes in service user health and wellbeing, care plans, risk assessments etc. are recorded and communicated across the team effectively.
Mentoring and role modelling to staff will also be part of the role.
This role includes day shifts, late shifts, sleepovers and nightshifts and weekend working all applicants must be able to do all shift patterns which includes public holidays, Christmas and New Year. All work will be community based, originating in the homes of the supported people within the service. Fife wide travel is an expected part of the role: although driving is preferred this is not essential. Travel costs incurred during work related travel will be reimbursed. Successful applicants will be expected to lone work for much of the day.
Successful applicants will be provided with all necessary training such as Physical Intervention and Escape Techniques, and you should be physically able to undertake this training as part of your role.
The role of Social Care Worker can be physically and emotionally demanding.

The Person
You must register with the Scottish Social Services Council (SSSC) within 3 months of taking up employment in this role.
You will be supported to achieve the qualifications required for practice, including the Social Services and Healthcare SCQF Level 7 (SVQ3\) or an equivalent award, within your initial registration period to meet Scottish Social Services Council requirements.
You will demonstrate good communication skills, effective organisational skills and the ability to manage and prioritise workload. You will have the ability to work with minimal supervision, role model team working skills, and have literacy skills and basic IT skills. You must have the ability to establish and maintain professional relationships in respect of service users, carers and colleagues by participating in open and sensitive dialogue.
Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.
As this role requires SSSC registration, you will have three months to apply for registration.
If appointed, you must start your application as soon as possible so you're registered within the required six months.
If you need to achieve a qualification to maintain your SSSC membership, information will be provided if you are appointed. You must ensure you complete the qualification within the specified timeframe.
