Social Care Coordinator
Salary: £34,434 to £38,220 per annum pro rata

We are looking for a Social Care Coordinator to join our friendly and supportive team working in the community in the lovely Yorkshire Dales.
The advertised vacancy is based in Belle Vue Square, Skipton, but you will be working from both your home and office base, and you will be provided with the appropriate technology to support you.
You will be working in an established Community Social Care Team, supporting people from age 18 to 75 years, including those who are physically or learning disabled and those with mental health services in place, as well as those in receipt of d funding streams such as Continuing Health Care. You will be completing assessments and support plans to meet people’s needs in a person\-centred way. You will be supporting people to remain living at home and promoting their independence.
You will have a good understanding of the Care Act 2014 and the Mental Capacity Act 2005 and the key principles underpinning the legislation. You will be able to put this knowledge into practice or willing to take responsibility for your development in these areas, where training and guidance will be provided. You will demonstrate the importance of dignity, respect, and professionalism in all aspects of the role. You will work closely in partnership with a variety of teams and agencies, including our health colleagues, stronger communities, housing, the Police, prevention teams and independent sector organisations.
We are a supportive team with a strong focus on peer support and team engagement and a focus on developing team members, there will be opportunities to undertake additional training such as practice education, if desired.
Previous experience of working within the Adult Health or Social Care sector would be preferable. You will either hold a Health and Social Care NVQ level 3 (Adults) or be willing to obtain this qualification if appointed.
