About the Role
This is a hybrid role combining hands\-on care delivery with field\-based coordination and quality oversight. Day to day, you will be delivering care to our clients across the Chippenham area whilst also taking responsibility for client assessments, care planning, and staff support.
For this role, we specifically need someone with hands\-on experience in risk assessment and needs assessment \- this isn't something you'll learn on the job, it's something you'll be doing from day one.
You will be well supported by our management team through regular check\-ins, supervision, and guidance. We are looking for someone who is confident in their own judgement, works well independently, and keeps management in the loop without needing to be asked.
What the Role Involves
- Delivering high\-quality, person\-centred domiciliary care to clients across the Chippenham area.
- Conducting client needs assessments and risk assessments to CQC standards.
- Writing care plans and carrying out regular client reviews.
- Keeping documentation accurate and up to date.
- Building strong relationships with clients, families, and health and social care professionals.
- Supporting junior carers and participating in the on\-call rota when needed.
- Working closely with our management team, who provide regular support and supervision.
- Minimum 2 years of experience in domiciliary or home care, including some supervisory experience.
- Hands\-on experience completing risk assessments and needs assessments \- essential, not preferred.
- NVQ / QCF Level 3 in Health and Social Care (Adults) or equivalent.
- Proven experience in completing care plans, risk assessments, and client reviews.
- Good knowledge of CQC standards and adult safeguarding.
- Confident working independently, with strong communication skills.
- Full UK driving licence and own vehicle with business use insurance.
- From £14\.35 per hour
- 20 guaranteed hours per week \- not zero hours
- 35p per mile for all business travel
