The role:

We are looking for a Band 6 Registered Nurse working within the North West Rehabilitation Service as part of a dynamic multidisciplinary team. The role focuses on assessment, treatment, and intervention for older adults and disabled adults with complex needs, promoting independence, selfmanagement, admission avoidance, and safe hospital discharge. You will independently manage a complex caseload and contribute specialist nursing expertise.

Duration, Location, and Working Pattern:

Fixed\-term, part\-time

This post is fixed\-term due to maternity cover

Shift Pattern \- 26 hrs over Monday \- Sunday

Base location \- Community Centre for Health

Key responsibilities:

  • Managing a clinical caseload independently, applying specialist nursing knowledge.

  • Delivering personcentred care focused on rehabilitation and maximising independence.

  • Working within a multidisciplinary team to support assessment, treatment, and discharge planning.

  • Supervising support workers and students.

  • Providing professional nursing leadership, guidance, and development within the team.
Knowledge, training, qualifications and/or experience required to do the job:
  • Must be an RCNregistered nurse.

  • Experience in community nursing and multidisciplinary teamwork

  • Relevant background in neurology
