An exciting opportunity has arisen for a band 5 RMN to join our newly developing Pennine Community Mental Health Rehabilitation Team.

LSCFT Specialist Mental Health Rehabilitation is currently undergoing transformation across inpatient and community services to implement a robust pathway for people with complex mental health needs in the least restrictive and safest way. There is a need to develop the range of service provision that will enable this. The aim of the transformation is to provide a service which can offer a whole pathway of support locally, identified in both national plans and clinical guidance, which would include an inpatient and community service offer that is matched against the identified need of LSCFT population.

We are looking to recruit an RMN that has a keen interest and a passion for nursing in community mental health, with post registration experience and experience in a relevant mental health setting. You will have excellent communication skills, sound clinical reasoning and a strong understanding of the nursing role within recovery.

You must have knowledge of current best practice in community mental health and awareness of relevant government directives and legislation.

You will receive regular supervision, access to CPD opportunities and be part of a supportive care group.

The post holder will work as part of a specialist mental health rehabilitation team that provides specialist assessment, treatment, the delivery of evidence\-based interventions and support to enable the recovery of people whose complex needs cannot be met by general adult mental health services. This service aims to work with people to help them acquire or regain the skills and confidence to live successfully in the community. This process can take many years and requires staff who enjoy working with patients who progress incrementally over a longer term. The focus is on addressing and minimising the symptoms and functional impairment that people may have, with an emphasis on achieving as much individual autonomy and independence as possible, whilst holding therapeutic optimism and celebrating service user gains to aid their recovery.

The post holder will provide hospital in\-reach and community\-based support to people with mental illness, significant comorbidities and complex social needs who need intensive community support, supported accommodation or inpatient rehabilitation. The community support will comprise the Pennine footprint including Blackburn with Darwin.

You will work alongside other colleagues of differing disciplines as part of an MDT approach to service user care, including in\-reaching to hospitals to support transition from hospital to home.

LSCFT values diversity and inclusion, recognising that talent from varied backgrounds creates a flexible, creative, and effective workforce. We actively challenge discrimination and welcome applications from all, regardless of age, disability (including those with experience of accessing or caring for someone who has accessed mental health or learning disability services), ethnicity, gender, religion, belief, or sexual orientation.

Our aim is to reflect the communities we serve, so we particularly encourage applications from under\-represented groups. If you’d like to discuss your application, please ask.

We also welcome individuals with lived experience relevant to the role or service area, as your insight brings authenticity to our services.

LSCFT support flexible working, we believe that a positive work/life balance benefits NHS employees through improved health and wellbeing. We would consider working patterns such as term time, part time, compressed hours and flexi\-time.

The wellbeing of our people is at the heart of everything we do. We offer a wide range of resources, tools and support to help you thrive both in and out of work.
