Hours: 30\-37 per week (can be negotiated at interview to work 37 across 5 days or approx. 30 across 4 days).

This is an exciting opportunity to lead innovative, place\-based work that integrates physical activity into health and care across Somerset's coastal communities. Working at the heart of neighbourhood health, you will develop and strengthen partnerships across the NHS, primary care, local government, voluntary and community organisations to create more connected, community\-centred approaches to supporting adults with long\-term health conditions.

You will lead projects that embed physical activity within health and care pathways, champion prevention and self\-management, and help shape sustainable models of care that reduce health inequalities, particularly within rural and coastal communities. Managing a small team and working collaboratively across organisational boundaries, you will drive system change, support falls prevention and musculoskeletal health initiatives, and ensure learning and evidence inform future development. This is a unique opportunity to influence how health, care and community services work together to improve the health and wellbeing of Somerset residents.
