Practice Nurse 18 Hours per week

We are looking for a caring and enthusiastic Practice Nurse to join our friendly, supportive and patient\-centred team for 18 hours per week due to our expanding patient population. The working days will be agreed with the successful candidate.

  • Friendly and experienced administration team supports the delivery of high\-quality patient care

The Practice is well supported by NHS Fife services, including:
  • A Mental Health Nurse

  • CTAC Services

  • An In\-Hours Urgent Care ANP

  • Pharmacy Team

  • District Nurses

Our multi\-disciplinary approach enables us to deliver high quality, patient\-centred care to our Patients and Community.

Key responsibilities for the position are:

  • Managing Long\-term Conditions including Diabetes, Asthma, COPD, Hypertension, CKD and CHD

  • Keeping up to date with Chronic Disease Practices and Guidelines.

  • Reviewing Chronic Disease Test Results (including bloods)

  • Delivery of Lifestyle Advice and Health Promotion

  • Ideally a Non\-Medical Prescriber

  • Attend and participate in Practice Meetings
