Job Overview

We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. In this vital role, you will deliver high\-quality patient care, support clinical procedures, and promote health and well\-being within the community. KEY RESPONSIBILITIES

· Administer adult and child immunisations and vaccination in accordance with national and local programs. Is competent in anaphylaxis and resuscitation techniques. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.

· Takes a lead in areas such as Asthma and COPD management, undertakes the establishment and running of disease management clinics and provides specialist nursing care for the patients or communities that practice serves.

· Management of chronic diseases which includes diabetes, asthma, hypertension, COPD,CVA/TIA, CHD and risk assessment and management.

· Maintain disease registers.

· Initiates and carries our programme of Health promotion and screening, cervical screening and swabs.

· Acting as Nurse Lead for infection Control, ensuring all infection control policies are implemented in the practice. (please see Health and Safety)

· Mentor and supervise Assistant Practitioner.

· Home visits for housebound patients.

Management

· Maintain accurate patient medical records, recording all consultations and procedures in accordance with practice policy.

· Shared responsibility for the day to day running of the nurses’ treatment rooms. Maintain tidiness and cleanliness.

· Ordering stocks and clinical equipment.

· Health education of patients and staff.

· Ensuring all infection control guidelines are implemented in the practice.

· Vaccine/cold change storage, monitoring and recording.

· Recognizes and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.

OTHER

· Attendance at practice and clinical meetings.

· Attendance at practice nurse forum meetings.

· Demonstrate an understanding of Child Protection.

· Updating present knowledge and attendance at educational courses as agreed with the practice.

· Assessment of services provided in the area and networking with other agencies.

· Promotion of self\-help groups.

· Effectively manage own time, workload and resources.

· Team work, supporting colleagues and communication with others.

· Maintain confidentiality.

· Promoting and maintaining own and others’ health and safety and security.

· Support the equality, diversity and rights of patients, carers and colleagues.

Health \& Safety

You will be expected to implement and lead on a full range of promotion and management their own and others’ health and safety and infection control as defined in the practice Health \& Safety policy, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

· Awareness of national standards of infection control and cleanliness and regulatory/contractual/professional requirements, and good practice guidelines.

· Responsible for the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.

· Management and maintenance of Personal Protective Equipment (PPE) for the practice, including provision, ordering, availability and ongoing correct usage by staff.

· Responsible for hand hygiene across the practice.

· Ownership of infection control and clinically based patient care protocols and implementation of those protocols across the practice.

· Active observation of current working practices across In relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses/training needs are identified, escalating issues as appropriate.

· Making effective use of training to update knowledge and skills, and initiate and manage the training of other across the full range of infection control and patient process.

· Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes ect are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.

· Safe management of sharps procedures including training, use storage and disposal.

· Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile and safe way, free from hazards. Initiation of remedial/corrective action where needed or escalation to responsible management.

· Actively identifying, reporting and correction of health and safety hazards and infection hazards immediately when recognised.

· Keeping own work areas and general/patient areas generally clean, sterile, identifying issues and hazards/risks in relation to other works areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector mangers.

· Undertaking periodic infection control training.

· Waste management including collection, handing, segregation, container management, storage and collection.

· Spillage control procedure, management and training. Decontamination control procedure, management and training, and equipment maintenance.

· Demonstrate due regard for safeguarding and promoting the welfare of children.

· In the performance of the duties outlined in this job description, the post\-holder may have access to confidential information relating to patient and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

· Information relating to patients, carers, colleagues other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

Pay: £17\.50 per hour
