<p> Job Responsibilities</p>
<ul>
<li>Perform one-time comprehensive medical need assessments in an individual’s home.</li>
<li>Provide expert recommendations for equipment, service, and care that empower members to maintain independence</li>
<li>Complete straightforward documentation through our secure, user-friendly online potable accessible from any device</li>
</ul>
<p> Qualifications and Requirements</p>
<ul>
<li>Current licensure as a Physical or Occupational Therapist (Sorry, No PTAs or COTAs).</li>
<li>Malpractice/Liability Insurance (may be purchased at the time of job offer).</li>
<li>Ability to lift up to 30 pounds, navigate stairs and varied home environments, and perform physical assessments as needed</li>
<li>Reliable transportation and the ability to travel to and from member homes</li>
<li>Proficient with technology: able to navigate online portals, upload documents, and use your preferred device (computer, tablet, or smartphone) for all work tasks</li>
<li>Strong critical thinking, problem-solving, and communication skills</li>
</ul>
<p>Preferred Experience (Not Required)</p>
<ul>
<li>Home safety assessment background</li>
<li>Knowledge (Familiarity) of durable medical equipment</li>
<li>Experience with home modifications</li>
<li>Work history with diverse medical conditions</li>
</ul>
