We are seeking a dedicated Transition Social Worker to provide comprehensive social care services to children transitioning to adult services who experience moderate to severe mental health problems or are dual diagnosed as autistic. This role requires a practitioner who can work collaboratively across disciplines, promoting a needs-based approach and developing clear support plans.

Day-to-Day Responsibilities

  • Assess individual needs with an emphasis on early intervention and prevention for those with complex needs, engaging individuals and carers in the process.

  • Develop and implement support plans following a Care Act assessment, including completing funding applications and legal work such as COPDOL, DOLS, and 21a’s.

  • Engage meaningfully with individuals often referred to as ‘difficult to engage’ and their families.

  • Support carers through the Care Act and work with children about to turn 18 to transition into adult services.

  • Maintain knowledge of SEND and EHCPs, ensuring compliance with support plans for individuals with complex needs.

  • Collaborate with other agencies including health, education, and housing to ensure a holistic approach to care.

  • Monitor and reassess the needs of service users, adjusting support plans as necessary.

Required Skills & Qualifications
  • Professional Social Work Qualification (Social Work Degree or DipSW).

  • Completion of ASYE.

  • Full current UK driving licence and access to a car.

  • Registration with Social Work England.

  • Substantial post-qualifying experience working with children transitioning to adult services and with mental illness.

  • Excellent written and analytical skills, with the ability to communicate effectively across a range of stakeholders.

  • Strong organisational and negotiation skills, with the ability to work in situations of conflict.

  • Knowledge of relevant legislation, policies, and procedures related to social work.
