We are seeking a dedicated Mental Health Social Worker to provide comprehensive social care services to adults of working age experiencing moderate to severe mental health problems within the community. This role is based in our Adult Services Community Mental Health Teams and requires a professional who can work collaboratively across disciplines, promoting a needs-based approach and developing clear support plans.

Day-to-Day Responsibilities:

  • Conduct Care Act assessments and formulate support plans, including completing funding applications and legal work such as COPDOL, DoLS, and Section 21A reports for court.

  • Engage meaningfully with individuals often referred to as 'difficult to engage', including their families.

  • Support carers through the Care Act and assist children transitioning to adult mental health services.

  • Work within a team that offers a 6-day service, primarily Monday to Friday with occasional Saturdays. Flexibility is required for planned interventions.

Required Skills & Qualifications:
  • Degree in Social Work (CQSW/DIPSW/CSS) with experience in adult social care and mental health.

  • Registration with Social Work England.

  • Full Driving Licence and completed ASYE.

  • Proven experience in social care and mental health practice, with skills in clinical supervision.

  • Strong risk assessment capabilities and the ability to manage unpredictable situations.

  • Proficient in written and verbal communication, with the ability to prepare and complete professional documents and reports.

  • Knowledge of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998.

  • Computer literacy and experience with electronic record systems.
