Job Summary
We are seeking a dedicated Part\-Time Experienced Care Coordinator to join our team. The successful candidate will be responsible for overseeing care plans, supervising staff, and ensuring the delivery of high\-quality care within a care home specialising in senior care. This role requires strong leadership, excellent communication skills, and the ability to coordinate effectively with residents, families, and healthcare professionals. The Care Coordinator will play a vital role in maintaining a safe, supportive, and respectful environment for our residents.
Responsibilities
- Develop, implement, and review personalised care plans tailored to each resident’s needs.
- Supervise and support care staff to ensure compliance with care standards and organisational policies.
- Coordinating and scheduling care visits efficiently
- Managing staff rotas and ensuring adequate care coverage
- Liaising with clients, families, healthcare professionals, and care staff
- Conducting care reviews, assessments, and care plan updates
- Monitoring service quality and supporting compliance with CQC standards
- Assisting with recruitment, onboarding, and staff support
- Maintaining accurate records and care documentation
- Responding to day\-to\-day operational and service delivery needs
- Assist with emergency procedures and ensure safety protocols are followed at all times.
- Previous experience as a Care Coordinator within domiciliary care or a similar health and social care setting
- Strong organisational and time\-management skills
- Excellent communication and interpersonal skills
- Good understanding of care planning, safeguarding, and compliance requirements
- Ability to work independently and manage multiple priorities
- Competent in using Microsoft Office and care management systems
- Up\-to\-date Mandatory Training certificates
- Right to work in the UK
- Enhanced DBS (or willingness to obtain one)
- Full UK driving licence and access to a vehicle desirable but not essential
