We are looking for a motivated and compassionate individual to join our Paediatric Diabetes Team, supporting the care of approximately 230 children and young people living with Type 1 Diabetes.

Working closely with our experienced Paediatric Diabetes Specialist Nurses, you will play an important role in delivering high‑quality, child‑centred care. Under supervision, you will support a varied caseload and, once trained and assessed as competent, take on routine and extended responsibilities that contribute to the smooth and efficient running of our Paediatric Diabetes clinics.

What You’ll Be Doing

Supporting the management of a caseload of children and young people with Type 1 Diabetes

Carrying out routine and extended clinical duties, in line with your training and competency

Working under the guidance of the nurse in charge and the Paediatric Diabetes Specialist Nurses

Helping ensure clinics run smoothly and efficiently, providing a positive experience for patients and families

Access to a car, as the role includes clinics in Frome and Chippenham

  • To take a lead role in the efficient and smooth running of Paediatric Diabetes Clinics at RUH, Frome Medical Centre and Chippenham Community Hospital. Car and full driving licence is required.

  • Preparing for clinics; ensuring all equipment is in good working order and ready for use; taking any relevant educational material; to set up ahead of time to ensure clinic starts on time

  • To perform HbA1c blood test for each child attending clinic using Point of Care testing equipment.

  • To maintain stock of equipment and materials and to ensure quality control and calibration of equipment.

  • Demonstrates good housekeeping techniques and correct disposal of sharps, reporting hazards, takes positive action to reduce any risks within own capability. Assist in the cleaning and maintenance of department areas, clinical equipment and stock levels. Reporting faults, breakages or missing items.

  • Undertake general clerical duties that may be associated with the smooth running of the department. Upkeep of notice and information boards.

Be a positive role model when contributing to the ongoing support of Children, Young People (CYP) and their families enabling them to maintain their interest, identity and emotional well\-being. Adapts to individual situation and modifies communication appropriately.

  • Presents a positive image and demonstrates good customer service, equality, respect, tolerance and sensitivity when dealing with patients/clients, their families and other visitors to the services.

  • Deal directly with queries from patients and other health care professionals and refer to appropriate diabetes team member where necessary.

  • Answers the telephone in a clear, friendly, helpful and professional manner. Takes messages accurately and refers to a PDSN as necessary.

  • Demonstrates a caring approach to CYP and their families. Have good verbal and non\-verbal communication skills. Acts as an advocate for CYP always.

  • Demonstrates competence in the Trust Electronic Patient record system DIAMOND.

  • Maintain high standards of care ensuring that all procedures and guidelines are followed.

  • Following training and assessment of competence undertake practical tasks as designated by the PDSN’s.

  • To take a lead role in the efficient and smooth running of Paediatric Diabetes Clinics at RUH, Frome Medical Centre and Chippenham Community Hospital. Car and full driving licence is required.

  • Preparing for clinics; ensuring all equipment is in good working order and ready for use; taking any relevant educational material; to set up ahead of time to ensure clinic starts on time

  • To perform HbA1c blood test for each child attending clinic using Point of Care testing equipment.

  • To download blood glucose meters, sensor glucose meters and insulin pumps using the appropriate downloading platforms and software.

  • To make follow up phone calls to families who did not bring their child to a clinic appointment and liaise with team administrator re follow up arrangements.

  • To be competent to do blood glucose and blood ketone checks and interpret readings.

  • To be competent to teach others to undertake blood glucose and blood ketone checking and insulin injections.

  • To assist Paediatric Diabetes Specialist Nurse (PDSN) in delivering diabetes training to other agencies.
Under direction from PDSN, and clear written plan, be able to give follow up support to children following High HbA1c Policy.
  • To undertake agreed clinical activities with patients under the direct and indirect supervision of the PDSN.

  • Flexible response to patient difficulty with technology or equipment.

  • To facilitate or supervise the insertion of sensors via sterile procedure and support Flash Glucose/Continuous Glucose Monitoring (CGM) and MyLife technology starts.

  • To demonstrate and support the setup of glucose monitoring systems, readers and apps.

  • To demonstrate how to video call.

  • To demonstrate how to download and send clinical information.

  • To make relevant and necessary decisions on substitute meters, finger\-pricking and glucose reading devices and analyse data and results accordingly.

  • To maintain stock of equipment and materials and to ensure quality control and calibration of equipment.

  • To take on responsibilities of the team administrator during annual leave and sickness to ensure a smooth running of the service.

  • Manage his/her own time effectively and perform allocated tasks within a reasonable time frame, knowing when to seek support, assistance and advice from other team members.

  • Undertake general clerical duties that may be associated with the smooth running of the department. Upkeep of notice and information boards.

  • Update computer information system ensuring accurate patient information and enter treatment changes into the Diamond diabetes database.

  • Demonstrates good housekeeping techniques and correct disposal of sharps, reporting hazards, takes positive action to reduce any risks within own capability. Assist in the cleaning and maintenance of department areas, clinical equipment and stock levels. Reporting faults, breakages or missing items.

  • Assist PDSN’s when preparing necessary documentation for courses run for other health care professionals and CYP and families.

  • Identify report and take appropriate actions as per level of competence for any untoward incident/accident, problem and risk as soon as possible to the Lead PDSN.

  • Be a supportive role model and contribute to the team as an active team member always demonstrating a flexible and co\-operative approach. To attend team and regional network meetings.

  • Participate with all team members in maintaining and monitoring standards and assisting with audits. Help to identify potential improvements within the environment.
