Pennine Care NHS Foundation Trust is pleased to advertise two permanent, full\-time opportunities for Mental Health and Wellbeing Practitioners to join our team working across the Oldham Primary Care Networks and the Oldham Neighborhood Mental Health Team.

This role is a key part of the ongoing transformation of community mental health services for adults and older adults, helping to bridge the gap between primary care and specialist mental health services,

The focus of the role is to provide early assessment, brief psychological interventions, and support individuals to access appropriate community and mental health services. You will help people to identify achievable goals, promote self\-management, and connect patients with relevant resources in the local area. You will provide short\-term psychological interventions, including group programs, under the direction of the principal clinical psychologist. You will play an integral role in the multi\-disciplinary team, supporting formulation and sharing expertise on appropriate psychological interventions.

To work alongside clients, families, carers and multi

disciplinary team members to co\-ordinate care, supporting

collaborative decision making about care and treatment.

To deliver a set of wellbeing\-focused psychologically informed

interventions aligned to cognitive – behavioural principles,

based on the best evidence available that address problems

often experienced by people severe mental health problems

who often who have experienced trauma.

To work under clinical supervision from a suitably qualified professional to provide high quality collaborative treatment as a member of the multi\-disciplinary team. To work under close clinical skills supervision from an HCPC registered clinical psychologist or BABCP accredited cognitive behavioural therapist to provide specified psychologically\-informed wellbeing\-focused interventions.

  • To deliver specified wellbeing focused psychologically informed interventions in line with best available evidence, under close supervision from a practitioner, psychologist or CBT therapist including:
o Behavioural activation and graded exposure o Teaching problem\-solving skills o Improving sleep/ dream completion o Recognising and managing emotions

o Guided self\-help for a range of conditions, including anxiety, disordered eating, low mood

o Building confidence
