Job summary
Are you a Registered Mental Health Nurse, Occupational Therapist or Social Worker who is passionate about helping people achieve meaningful recovery and live well within their communities?
This is an exciting opportunity to join a forward\-thinking Community Mental Health Service that is transforming the way mental health support is delivered. Working within a collaborative and recovery\-focused team, you'll support people with severe and enduring mental health difficulties, helping them build on their strengths, increase independence and achieve their personal goals.
You'll manage your own caseload, deliver evidence\-based interventions, and work alongside individuals, families, carers and partner organisations to provide holistic, person\-centred care. As an experienced practitioner, you'll also play an important role in shaping service development, supporting colleagues and contributing to the ongoing improvement of mental health services.
Please note that this role requires a full driving licence and access to your own vehicle with appropriate business insurance, as travel across the local community is an essential part of the position.

Main duties, tasks \& skills required
This role offers the chance to make a genuine difference every day. You'll work autonomously while being supported by an experienced multidisciplinary team, using your professional expertise to deliver compassionate, recovery\-focused care across community settings.
You'll undertake comprehensive assessments, identify strengths and needs, and develop personalised plans that empower people to take an active role in their recovery journey. Using your knowledge of evidence\-based practice and psychosocial interventions, you'll help people manage their mental health, build confidence and maintain their wellbeing within their local communities.
Working across organisational boundaries, you'll collaborate with GPs, inpatient services, social care teams, housing providers, voluntary sector partners and other agencies to ensure individuals receive coordinated, joined\-up support. Strong communication and relationship\-building skills will be key as you navigate complex situations and advocate for the needs of those you support.
As a senior member of the team, you'll have opportunities to develop your leadership skills through supervision, mentoring and supporting less experienced colleagues and students. You'll also contribute to shaping and improving services, helping to ensure the team continues to provide high\-quality, innovative care.
