Memory assessment Nurse
A 12 month secondment opportunity has arisen for a suitably qualified and experienced Registered Mental Health Nurse (RMN / CPN) to join a busy Memory Assessment and Treatment Service in Trafford based at Cross gate House, Sale, Trafford.
The successful candidate will be required to fulfil the role of a memory nurse within an MDT. The successful candidate would be expected to complete screening of new referrals via the established duty system, offer titration / post diagnostic support as outlined in the SOP. There will be opportunities to liaise and work with partner organisations in ensuring that the service manages all referrals in a timely manner in line with the service standard operating procedures (SOP).
We are ideally looking for a passionate practitioner who has a sound knowledge of the different types and presentations of dementia and experience in working in a community setting. The candidate would be required to demonstrate sound risk management abilities and be able to formulate risk management plans for individual clients seen in our settings. We would also expect the successful candidate to have excellent communication skills both within the team and with clients and carers.
In return our service offers regular line management and clinical supervision as well as support with relevant training. There will be opportunities to further enhance personal knowledge and skills within the sphere of clinical practice.
- The post\-holder will undertake assessment and treatment of clients referred for investigation of possible mild cognitive impairment and dementia, in accordance with NICE Guidelines and evidence\-based practice, to support the work of the MATS service.
- To act as Duty Officer completing the screening of referrals, offering advice and signposting.
- The post\-holder will contribute to the development and delivery of an individually tailored service for people with dementia.
- The post\-holder will take an active role within the relevant multi\-disciplinary teams and will work within these services co\-ordinated frameworks.
- The post\-holder will contribute to audit and research work undertaken by the MATS Team.
