We are excited to offer a new opportunity for a Learning Disability Community Case Coordinator to join our team on a 12\-month pilot basis.
This is a unique role working across our community residential services, supporting adults with learning disabilities and/or autism. You will play a key part in improving how our services work together helping ensure people receive coordinated, timely, and person\-centred care.
Working closely with Residential Managers, support teams, and Specialist Nurses, you’ll act as a central point of coordination—helping to connect services, streamline communication, and support positive outcomes for the people we support. What You’ll Be Doing:* Supporting coordination of care across community residential services
- Acting as a key link between support teams, healthcare professionals, and external services
- Gathering and organising information to support referrals, reviews, and care planning
- Attending and contributing to meetings (including Care \& Treatment Reviews)
- Assisting with assessments, observations, and record\-keeping (under supervision)
- Building positive relationships with the people we support and their families
- Helping ensure communication is clear, timely, and effective across all teams
We’re looking for someone who is organised, compassionate, and proactive, with a genuine passion for supporting people with learning disabilities. You will ideally have:* NVQ Level 3 in Health \& Social Care (or working towards it)
- Experience supporting individuals with learning disabilities and/or autism
- Strong communication and organisational skills
- Confidence working with multiple teams and services
- Ability to manage your own workload and prioritise effectively
- A person\-centred and empathetic approach
- Supportive multidisciplinary team environment
- Ongoing training and professional development
