Lead Wound Healing Nurse, Community Services
We are seeking an experienced and inspiring nurse leader to transform wound care across community and integrated services. This strategic role will lead service development, improve patient outcomes and experience, and drive innovation and integrated care pathways across neighbourhood teams and partners. The successful candidate will be a Registered Nurse with expertise in tissue viability, strong leadership and service improvement experience, and a passion for delivering high‑quality, patient‑centred care in the community, aligned with UHCW values.
The group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. This pilot aims to explore team structures and assess the impact on patient experience. While no formal redesign has taken place yet, the outcomes of this trial may inform future service delivery and team configuration across the group.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
Provide leadership for tissue viability services, ensuring high‑quality, evidence‑based care. Work with multidisciplinary teams to improve outcomes, reduce pressure ulcers, support innovation in wound care, and lead education and quality improvement initiatives across the organisation

Provide senior professional leadership and corporate accountability for tissue viability services across Integrated Care and Community Services.
Lead the strategic clinical governance framework for wound care, ensuring alignment with PSIRF, quality surveillance, risk management and assurance reporting.
Act as the expert advisor within community group and system delivery on tissue viability, influencing strategic decision‑making, pathway standards and policy development.
Analyse and interpret complex clinical intelligence, performance data and trends to inform strategic planning, service assurance and risk mitigation.
Identify and manage clinical and operational risks, escalating issues appropriately and providing expert advice to senior leadership.
