Job description
To provide support and assistance to people in accordance with their support plans and service aims. Be responsible for initial and on going assessments of person/people who have complex or multiples needs. Offer support \& assistance to people in crisis situations and/or manage physical behaviour likely to cause incidents in accordance with the support plan or service protocol in accordance with guidance from senior colleagues or service aims. Must have a good understanding of the causes and effects of social exclusion. Able to travel within the service area you are contracted for, with actual experience of carrying out comprehensive alcohol and drug assessments to carry out drop in one day on a rota and providing 1:1 keyworker psycho social support service to a caseload of 30\-35 adults with alcohol or drug issues.
Must have knowledge of both harm reduction needle exchange , bbv testing routes transmission , knowledge of specific alcohol and drug harms, relapse prevention and recovery systems of care capable of making referrals to tier 2 groupwork and rehab and familiar with Recovery Outcomes wheel similar to outcome star.
Must have good IT skills and be able to use electronic database, able to record high quality case file recording.
Any knowledge Motivational Interviewing helpful and understanding trauma informed.
Good opportunity to work in co located recovery hub with NHS colleagues provide clinical treatment . Very much multi disciplinary approach and holistic, refer people on foodbanks , benefits, housing advice , housing support , social care direct , debt advice counselling etc
It is essential applicants have a minimum of 6 months previous experience in support work in the UK , must have achieved SVQ 3 qualification in Health \& Social Care.
All applicants will be subject 2 professional reference checks and must already be/willing to become a member of the PVG scheme and register with SSSC.
