Job Purpose

The main purpose of the role is to ensure the efficient and effective delivery of a trauma informed and recovery\-based model within our specialist mental health accommodation. The post\-holder will provide support to residents with complex needs including mental health, forensic, drug addiction, alcohol issues and physical health needs. They will be responsible for ensuring a high\-quality service is delivered on a 24\-hour basis. The post holder will take responsibility for a case load of residents which is outcome focused and responds to the needs of residents.

Job Summary

The role will require some knowledge in mental health, and will work within the following areas:

\- The main purpose of the role is to lead shifts, and key work a case load of residents.

\- Ensure a high quality, recovery based, trauma informed service is delivered on a 24\-hour basis.

\- Where the service is registered with CQC to support the manager to ensure that all CQC regulations are upheld.

\- Provide a consistent approach to resident support.

\- Working in partnership with other agencies and our local communities.

\- Ensure effective systems to promote both safety and security are deployed.

\- Demonstrate a positive and objective attitude and behaviours in everyday practice.

\- Ensuring that key performance targets are met and that all resident records are up to date

Main duties and Responsibilities

This job description is not definitive and will be reviewed periodically in the light of

Service Delivery

\- Provide a consistent approach to resident care and support.

\- Ensure that recovery and Trauma informed approach is effectively deployed in a compassionate and respectful manner

\- Ensure the smooth running of the day\-to\-day service running.

\- Ensure that move on and developing independence are at the fore front of service delivery.

\- Ensure that SMOT Co\-production agenda is at the heart of services.

\- Comply with all relevant St Martin policies \& procedures, including but not limited to: Safeguarding, Health \& Safety, resident Care, Equality \& Diversity, the Code of Conduct etc.

\- Ensure incidents are always reported using the Datix system.

\- Provide day\-to\-day shift leadership which includes organising the shift and delegating to other staff members.

\- Implement new ideas for the improvement of services in conjunction with colleagues.

\- Ensure effective systems to promote both safety and security are deployed.

\- Work across the services as required and ensure environments are maintained in a clean and orderly manner.

\- Ensure high levels of hygiene and infection control systems are maintained.

Care and support of residents

\- Develop and implement care and support plans to meet residents’ needs.

\- To be the Keyworker to a case load of residents; developing and evaluating support/care plans and carry a caseload.

\- Complete care and support plans and update risk assessment as needed and within KPI timelines.

\- Complete key work session weekly for all residents.

\- Use SMART goal planning to provide needs led holistic support

\- Carry out assessment to identify and prioritise needs at referral stages and throughout support offered.

\- To support residents with their medication according to best practise and St Martin’s policy.

\- Ensure that all residents are supported to develop independence and work towards move on where appropriate.

\- Engaging with residents daily to build their trust and encourage them to take part in various activities within the service and wider community.

\- To escort them to appointments, activities and attend meetings in community.

\- To attend CPA and other professional meetings for key resident.

\- To support residents with personal hygiene and toileting, along with dressing and maintaining their personal appearance in our CQC services.

\- Undertake to support a proactive regime involving residents in a range of activities.

\- Ensure that the residents’ rights and dignity are maintained.

\- Manage and escalate concerns regards care and treatment and promote the whistleblowing policy so staff are aware of how it works.

\- Ensure confidentiality for resident, staff and the organisation are maintained.

Communication

\- Communicate effectively with all members of the team.

\- Establish effective, transparent and open lines of communication with care teams.

\- Demonstrate role modelling of St Martins values and act as an ambassador for our services.

\- Communicate the needs of residents effectively through handovers and emails to the rest of the team.

\- Ensure care notes, health and safety records are recorded accurately and on time.

\- Demonstrate a positive and objective attitude and behaviours in everyday practice.

\- Listen to staff and develop a culture that embraces new ideas.

People management

\- Actively engage in annual appraisals and competency assessments.

\- Manage and lead shifts as identified in shift leader protocol.

\- Ensuring compliance with HR processes including holidays/sickness absence etc.

\- Take responsibility for completing mandatory training.

Stakeholder management

\- Liaise with local partners and voluntary sector organisations to get the best mix of services for St Martin’s residents.

\- Ensure there is a safe and welcoming environment for staff, residents and visitors.

\- To regular liaise with stakeholders to increase referrals to the service and to present SMOT in positive light.

\- Offering support to internal stakeholders and completed requests timelessly

\- Attend any Multi\-Disciplinary Team meetings and promote St Martins in a positive light.

\- In collaboration with colleagues encourage others in creating and embedding high quality services.

Project financial management and budgets

\- Support the budgeting process and ensure that accurate records are held.

\- Support residents with budgeting their monies and adhering to policy and procedures.

\- Ensure that residents pay service charges.

\- Monitor and control spend to ensure alignment with the assigned budget.

Other

\- Complete any reasonable requests, including deputising for manager, senior staff and/or supporting colleagues in different services when required.

Job Type: Full\-time

Pay: £27,000\.00\-£28,000\.00 per year
