Inpatient Rehabilitation Nurse

Skylark is a 30\-bed community rehabilitation unit in Yate, South Gloucestershire, supporting patients on Discharge to Assess Pathway 2\. We are committed to helping people regain their independence and achieve the best possible outcomes following illness, injury or surgery.
Working as part of a skilled multidisciplinary team, you will provide person\-centred nursing care and rehabilitation to patients with a wide range of needs, including frailty, orthopaedic conditions, post\-surgical recovery and long\-term neurological conditions. The unit nurse is responsible for the assessment, planning, implementation, and evaluation of care for a group of identified service users and will provide and maintain a high standard of patient focussed care that ensures patient safety; dignity and respect are paramount to the care delivered.
Provide, monitor and ensure that a high standard of safe and effective nursing care is maintained via professional actions, verbal and written skills.
With the support of senior staff be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort.
Carry out a range of routine and non\-routine tasks including medication rounds, clinical observations, planning, implementing, and evaluating care plans, assisting patients to meet their daily care needs, health promotion, venepuncture, planning discharges for patients.
Work Pattern
You will be expected to work weekends and bank holidays on a rostered basis. Day and night shifts but predominantly day shifts.
