Family Doctor / General Practitioner (GP) – Excellent Opportunity in Carlow
Our client is seeking an experienced, patient\-focused General Practitioner to join our growing primary care team on a full\-time basis.
This is an exciting opportunity for a GP with extensive experience within the Irish healthcare system to lead the delivery of high\-quality, patient\-centred care across both the practice population and our associated nursing home services. The successful candidate will play a key leadership role in the development and implementation of the practice's Chronic Disease Management (CDM) Programme and other HSE\-funded and GMS initiatives.
Working as part of a multidisciplinary team, you will contribute to the continued growth and strategic development of primary care services within the practice and its wider network.
Key Responsibilities
- Provide comprehensive, evidence\-based primary care services to GMS and private patients.
- Deliver ongoing medical care and clinical oversight to residents in partner nursing homes.
- Conduct scheduled nursing home visits and additional visits as clinically required.
- Lead the practice's HSE Chronic Disease Management (CDM) Programme, ensuring achievement of programme targets and compliance requirements.
- Support and implement HSE and GMS initiatives, including preventative care and chronic disease pathways.
- Develop and manage patient\-centred treatment plans for acute and chronic conditions.
- Work collaboratively with nursing staff, allied health professionals, administrative teams and external healthcare providers.
- Maintain accurate, timely and compliant clinical documentation.
- Participate in clinical governance, quality improvement and audit activities.
- Support the strategic development and growth of the practice's community\-based healthcare services.
Applicants must meet all of the following criteria:
- Medical degree (MB BCh BAO, MBBS or equivalent).
- Specialist Registration in General Practice with the Irish Medical Council.
- Fully eligible to provide services under the General Medical Services (GMS) Scheme.
- Minimum of five years' post\-training experience working within Irish General Practice.
- Demonstrated experience managing GMS patient populations.
- Experience delivering or leading HSE Chronic Disease Management (CDM) Programmes.
- Strong understanding of HSE primary care initiatives, GMS contracts and chronic disease pathways.
- Excellent clinical, communication and organisational skills.
- Ability to work independently and collaboratively within multidisciplinary teams.
- Full clean driving licence and access to own transport.
- Experience in nursing home medicine, elderly care and complex chronic disease management.
- Experience leading service development and quality improvement initiatives.
- Interest in population health management and integrated community care.
- Competitive salary of €114,200 – €135,000 per annum.
