Directorate Manager (Integrated Community Services)
We have well established Integrated Community Services in Bridgend delivered in partnership between Cwm Taf Morgannwg UHB and Bridgend CBC. Our multi\-disciplinary services work closely with primary care, health and social care services and community partners to deliver the best health and wellbeing outcomes. This is a new senior role to build on our existing model and lead our community services into the future. We are seeking an experienced health and or social care professional to lead and drive our vision and continue the transformation and modernisation of integrated health and social care services.
We wish to make a joint appointment for a compassionate and collaborative leader who is committed to delivering high quality outcomes. This is a pivotal role responsible for operational management, shaping and delivering high\-quality, person\-centred integrated services for older people and individuals living with long\-term and chronic conditions. The post holder will work collaboratively across organisational boundaries to deliver seamless, integrated care that meets the needs of local communities. Being equally passionate in enhancing staff experience and wellbeing and able to influence at different levels within our organisations.
- Have operational responsibility for the performance of community services within the county, contributing to the development and preparation of plans for the achievement of national and local priorities.
- Provide leadership and management combined with operational responsibility for the resources and staff within the community services for the county.
- Enable effective delivery of services which reflect local community and stakeholder influence and the Health Board’s and Council’s vision, supported by staff located in the community setting.
- Ensure that Health Board and the Council aims and objectives and County targets and strategies are clearly communicated to all community\-based staff so that a full contribution can be made to implementation of operational plans reflecting these.
- Direct, lead and motivate staff within the community to ensure a high standard of professionalism, efficiency and effectiveness in service delivery, ensuring that activity is aligned to the Health Board and the Council plans and strategies.
- Undertake coaching, mentoring, resource planning, standard setting, performance management, team and individual development to ensure a culture of continuous improvement and professional excellence is achieved.

Service Management
- Be responsible for the management of staff and resources for community\-based services and the development and delivery of effective and efficient services for the county.
- Provides leadership, strategic combined with supportive operational responsibility for the resources and staff within community services across the county.
- Take the lead role but work collaboratively within the triumvirate model to deliver quality, performance, budget and good governance across the service.
- Ensure robust systems for Professional leadership are in place for all professions working within the locality community services.
- Foster a culture and practice of coproduction in the service area.
- Enables effective delivery of services which reflect local community and stakeholder influence and Health Board and the Council vision, supported by staff located within the corresponding facilities.
- Provides innovative proactive advice and support to a range of organisations, managers and staff in overseeing the provision of the services.
- Contribute to the Health Board’s and Council’s corporate annual plan, self\-assessment and corporate performance assessment process.
- Ensures that the Health Board’s and the Council’s aims and objectives are clearly communicated to all community staff so that a full contribution can be made to implementing the Health Board’s strategic and operational plans in the county.
- Undertake long term strategic planning for county wide community services provision to ensure they fit within overall Health Board and the Council’s strategic plans.
- Lead transformation/modernisation projects that deliver improved outcomes for individuals and more efficient and effective public services.
- Regularly monitor and review plans, adjusting for changes in local need and in response to changes in national policy guidance and the Health Board’s and Council’s strategies.
- Lead and sponsor projects to develop new ways of working in line with the Health Board’s and Council’s strategies and business plans.
- Investigate and deal with complaints arising within community services as required and in accordance with statutory guidance for complaints and representations of the Health Board and the Council.
- Negotiate with other senior health professionals, the Council’s Heads of Service and partners on a regular basis in relation to service issues, challenging practice on actions.
- Will work independently within local and national strategy, policy and guidelines, interpreting those to inform local policy development.
- Ensure the delivery, review and monitoring of the actions, required to deliver local and national Improvement Plans.
- Ensure compliance with safeguarding legislation and policy, and highest standards of safeguarding practice, in the Health Board and the Council.
- Ensure the delivery of Health and Safety requirements for staff, patients and visitors.
- Will be expected to support the relevant on\-call rota once identified.
- Ensure that the activities within community services are effectively delivered in accordance with the agreed Service Level Agreements (SLAs) and procurement frameworks.
- Ensure Community Services are successfully delivered to the highest standards and efficiency.
- Ensure that all agreed local and national targets are met.
- Effectively translate service improvement plans into concrete working practices which improve the efficiency and quality of care to patients and contribute to the overall achievement of the Health Board’s and Council’s objectives.
- Work with the Service Director Primary Care and Community Services and Head of Adult Service to ensure operational plans are achievable and core to service planning and delivery.
- To communicate effectively and establish good working relations.
- Build and maintain strong influential relationships with colleagues, staff and internal and external stakeholders e.g. Third Sector.
- Ensure that the Health Board’s and Council’s aims and objectives are clearly conveyed to all staff within the community services setting within the county.
- Develop close working relationships with Trade Unions and professional bodies to enable delivery on business objectives and financial targets.
- Provide training, presentations and information to a range of groups comprising staff, members of the public, senior staff and colleagues from partner organisations. The subject matter may include service changes or financial targets requiring negotiation or agreement.
- Participate in and facilitate meetings at a variety of locations within and outside of the Health Board and Council.
- To ensure the agreed performance data set is reported regularly into the Head of Adult Social Care and Service Director for Primary Care and respective reporting structures.
- Ensure high standards of information governance are implemented across the service area.
- To ensure there are robust systems and processes in place for maintenance and removal of records and office administration.
- Ensure up to date records are maintained of staff absence returns, annual leave, sickness records and attendance sheets and that returns are made to payroll as required.
