Care Manager

Location: Leicester, with travel across group services

About the role

We support people with complex needs to live well in their own homes, and the quality of that support comes down to the person holding the service together day by day. That is what this role is.

As Care Manager you own a service. You know the people we support by name, you know what a good day looks like for each of them, and you know when something is drifting before it becomes a problem. You lead a staff team, you keep the paperwork honest, and you are the person families, social workers and commissioners call when they want a straight answer.

This is not a desk job with a care label on it. You will be in services, in reviews, in supervisions.

What you will be responsible forThe people we support

  • Assessing new referrals and writing care plans and risk assessments that reflect the person, not a template

  • Reviewing plans as needs change, and making sure the team actually works to them

  • Leading mobilisation of new packages, from initial assessment through to a settled first month

  • Attending reviews with social workers, CHC teams, health professionals and families, and representing the service credibly

  • Making sure outcomes are recorded and evidenced, so funding conversations are grounded in real data
The team
  • Line managing support workers and senior support workers, including supervisions, appraisals, spot checks and probation reviews

  • Building rotas on Nourish Rostering that give continuity to the people we support and fairness to the staff

  • Holding difficult conversations early, including performance and conduct issues, with HR support

  • Supporting recruitment, onboarding and induction through our JotForms based recruitment and training workflows

  • Making sure training is current, competencies are signed off, and no one is working outside what they are trained to do
Safety and compliance
  • Acting as a safeguarding lead for your service, raising, tracking and following through on concerns

  • Managing incidents and accidents, investigating properly, and closing the loop with actions that stick

  • Maintaining records on Nourish so that what is written matches what happened

  • Running audits (medication, care plans, records, environment) and acting on the findings

  • Contributing to CQC readiness, provider information returns, inspection responses and action plans

  • Responding to complaints openly, honestly and in line with the duty of candour
The wider business
  • Working with commissioners and local authority frameworks, including tender and DPS submissions where needed

  • Flagging funding gaps, unfunded hours and package changes to the operations team early

  • Reporting weekly against agreed measures, so leadership can see where the service really is
Who we are looking for

Essential

  • At least two years of experience in care, with at least one year supervising or managing a team

  • Level 3 Diploma in Health and Social Care, or working towards Level 5

  • Solid understanding of CQC fundamental standards, safeguarding, the Mental Capacity Act and person centred practice

  • Confident writing care plans, risk assessments and incident reports that would stand up to scrutiny

  • Comfortable with digital systems and able to learn ours quickly

  • Full UK driving licence and access to a vehicle

  • Right to work in the UK

  • Enhanced DBS on the update service, or willingness to obtain one
Desirable
  • Level 5 Diploma in Leadership for Health and Social Care

  • Experience in supported living or complex care in the community

  • Experience with Nourish or a comparable digital care records system

  • Experience of local authority commissioning, reviews or tenders
