A customer experience lead is responsible for coordinating and overseeing the care and support provided to individuals with complex health needs. Their main objective is to ensure that the different aspects of a person’s care are well\-integrated and effectively delivered. They are also responsible for coordinating and liaising with various healthcare and social care professionals to ensure seamless care delivery and continuous care services. Additionally, they develop personalised care plans in collaboration with individuals they support, their families, and other relevant professionals.
- Salary \- £27,821\.04 pro rata
- Contracted 37 hours (7am \-1pm and 4pm\-10pm)
