Community Wellbeing Practitioner

Location: East Riding of Yorkshire (Community\-based with hybrid working) \-Goole, Haltenprise area

Hours: 14 hours a week

About the role

Wellbeing For Us is looking for a compassionate and motivated Community Wellbeing

Practitioner to support adults experiencing mental health and wellbeing challenges across

the East Riding of Yorkshire.

Working within a friendly and supportive team, you will help people improve their wellbeing,

build confidence and independence, and connect with opportunities within their local

communities. Through one\-to\-one support, wellbeing groups, workshops and partnership

working, you will empower people to achieve positive and lasting outcomes.

This is a varied and rewarding role for someone who enjoys working both independently and

collaboratively, building relationships within local communities and making a real difference

to people’s lives.

Main Responsibilities

The successful candidate will:

● Provide strengths\-based one\-to\-one support to adults experiencing mental health

and wellbeing challenges, including individuals with Care Act eligible needs.

● Complete assessments, support plans, reviews and referrals in line with

organisational policies and contractual requirements.

● Support individuals to develop independence, improve wellbeing and access

community resources, services and opportunities.

● Facilitate and deliver a range of wellbeing groups, workshops and recovery\-focused

courses within community venues across East Riding.

● Encourage the development of peer support and sustainable community

connections.

● Build and maintain positive relationships with statutory, voluntary and community

organisations to improve referral pathways and collaborative working.

● Attend partnership meetings, networking opportunities and community events to

promote the service and increase awareness of available support.

● Provide information, advice and guidance by telephone, email and face\-to\-face.

● Maintain accurate, timely and confidential case records using the organisation’s case

management system.

● Collect and record service activity and outcome data to support monitoring and

service evaluation.

● Contribute to the ongoing development and improvement of the service.

● Work in accordance with safeguarding, confidentiality, equality and data protection

legislation and organisational policies.

About You

Essential

● Experience supporting adults with mental health or emotional wellbeing needs.

● Knowledge of recovery\-focused, strengths\-based and person\-centred approaches.

● Experience facilitating groups, workshops or community activities.

● Excellent communication and interpersonal skills.

● Ability to build effective relationships with community organisations and partner

agencies.

● Experience completing assessments, maintaining accurate records and managing a

varied caseload.

● Good organisational and time management skills.

● Confident IT skills, including Microsoft Office.

● Understanding of safeguarding adults, confidentiality and data protection.

● Ability to work independently while contributing positively to a supportive team.

● Full UK driving licence with access to a vehicle.

Desirable

● Knowledge of the Care Act 2014 and adult social care.

● Experience working within community mental health, social prescribing, voluntary or

community sector services.

● Experience delivering psychoeducational or wellbeing programmes.

● Mental Health First Aid, ASIST or other relevant mental health qualifications.

● Knowledge of local community assets and support services.

Pay: £24,420\.00\-£26,787\.00 per year
