Community Staff Nurse

We are offering an exciting opportunity to join our supportive and friendly Community Nursing Ambulatory clinic team based across Cannock and surrounds as a Band 5 NMC Registered Nurse.

We would love to hear from you if you are passionate about being part of a team delivering the highest standards of wound care, tissue viability and catheter care to patients within a clinic setting and occasionally within their own home.

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence\-based practice.

You will holistically support patients to manage their long term conditions and care needs within the community and support patients with tissue viability needs and catheter management.

You will be required to travel independently across a geographical area.

As band 5 Community Nurse, you will have a commitment to providing holistic care to patients in a clinic setting and be able to work on your own initiative and work well within a team.

As part of this role, you will be expected to:

• Assess new service user referrals and care/support needs.


• Provide care directly to patients on the caseload with a range of nursing needs (complex wound care, pelvic health, medication administration).


• Promote independence and support patient centred care planning.


• Work autonomously, in line with your scope of practice, competence and policies.

Handover patient information to the wider team, document patient information effectively.

• Escalation of patients’ needs as appropriate.

We have around 9,000 staff who provide physical and mental healthcare, support for people with learning disabilities, and adult social care across Staffordshire, Stoke\-on\-Trent, Shropshire, and Telford \& Wrekin. We also run regional and national services, including help for new parents (perinatal care), eating disorders, forensic services, sexual health, and support for people in prison with mental health, drug, or alcohol problems.
