An exciting opportunity has arisen for a band 6 nurse to join the diabetic specialist nursing team. These specialist nurses are based throughout the trust however provided specialist advice for all specialities within the Trust .

The successful candidate will be required to undertake clinical assessments for our patients and liaise with the wider MDT regarding ongoing care and treatment plans. We strive to deliver a high standard of care to our patients ensuring that safe and timely care is provided at all times enabling both the patients and the the MDT to manage the acute / chronic conditions in the appriopitate environment.

We are looking for enthusiastic and motivated individuals who have an interest in diabetes.

We will provide specialist training as well as the support from the MDT team. Successful candidates must be a minimum of 2 years qualified or have diabetes experience.

The Nurse Specialist will provide support for the patient and family from diagnosis through

treatment and throughout the diabetes journey. Using advanced communication and

psychological assessment skills the CNS will aim to assess and provide management and

education for diabetes patients and their families, including identification of need for

onward referral to other agencies and disciplines as appropriate

Provide education and specialist advice to patients, their carers and staff on the specific

care and treatment for the speciality

Acts as a valuable resource for the multi\-professional team.

Set standards of care, influence the work of other professional groups and act as a change

agent

Work as a core member of the MDT supporting the diabetes pathway to improve the

patient experience.

Key Responsibilities

Leadership

  • Is actively involved in the performance management of diabetes to ensure patients have
prompt access to diagnostics and treatments.
  • Acts as a core member of the MDT ensuring the patient pathway is managed as
effectively as possible preventing un\-necessary delays in diagnosis and treatment.
  • Actively participates in multi\-professional meetings, acting as patient advocate and
representing nursing views
  • Working with the MDT is instrumental in supporting the implementation of the National
diabetes strategy at a local level including Holistic Needs Assessments
  • Supports the development of patient focussed education, including training to self
manage ongoing consequences of treatment.
  • Promotes the philosophy of person\-centred care, co\-creating care with people affected
by diabetes, embedding patient and public involvement within the sphere of practice
  • Working collaboratively with colleagues the CNS will promote compliance with National
Diabetes Audit (NDA) measures including annual and operational policy

  • Collates and records quantitative and qualitative data to provide evidence of
productivity, outcomes and quality, through audit and research.
  • Supports the implementation and monitoring of adherence to relevant clinical
guidelines.
  • Is responsible for ensuring effective communication between all service providers.
Clinical
  • As a core member of the MDT works with the team to support the best possible patient
outcomes.
  • Delivering evidence based individualised and personalised care through holistic
needs assessment, planning and evaluation of care and all care interventions.
  • As key worker ensure management plan to address key concerns which is shared
with patient, GP and any other relevant HCP
  • Actively integrates theory and practice

  • Acts as a Key Worker for a clinical caseload, working to coordinate care, share
management plans with relevant patient and providers and provide easy access for

ongoing support for patients.
  • Enables people to access, identify, appraise and interpret information, to determine
their own future.
  • Provides clinical specialist advice and support across the organisation.

  • Contributes to service development, ensuring the pursuit of excellence in care.

  • Makes clinical judgements based on experience and knowledge and take appropriate
action
  • Request diagnostic tests within agreed sphere of competence and make clinical
judgements within competence boundaries
  • Communicates sensitive information to patients/carers and professionals such as
breaking bad news, imparting news chronic life limiting conditions of type 1 and type 2

diabetes/ need for insulin for life or in some cases risk of mortality relating to their

condition and dealing with the emotions that arise.
  • Defines a clear care pathway, refers to other professionals and signposts to other
agencies as appropriate.
  • Promotes self\-management, undertaking risk stratification to determine those who can
self\-manage, following education, those who will need guided support to self\-manage

and those who will require on\-going, face to face support.
  • Assess and manage symptoms, problems related to area of expertise and recognising
when appropriate for onward referral is required.
  • To facilitate the assimilation of complex clinical information regarding diagnosis,
investigations and treatment options
  • Support and empower patients and carers to influence and participate in often difficult
decisions regarding the planning and delivery of care
