Clinical Nurse Specialist (CNS)/Candidate CNS (cCNS) \| INCLUSION HEALTH SERVICE

Permanent Contract, Full Time Hours.

The CNS/cCNS Inclusion Health will:

• Have nursing line management, reporting and professional relationship, accountable to the Director of Nursing.


• Work collaboratively with the ANP (Inclusion Health) to promote integrated working across acute and community services and sites.


• Have an operational clinical working relationship will be with the Consultant leading on the Inclusion Health Service.

• Mercy University Hospital (MUH) has established an Integrated Inclusion Health Service that effectively connects the Hospital and Community.


• This service addresses the multifaceted healthcare challenges and health inequity that occur for individuals at risk of social inclusion such as those experiencing homelessness, people affected by addiction, including harmful use of drugs or alcohol, members of the Travelling community, refugees and prisoners, people living with HIV or other communicable diseases.


• They liaise with appropriate services for people with dual diagnosis of mental illness and addiction.


• Through this service a seamless and collaborative healthcare network provides comprehensive and person\-centred care.


• The development of this service is directly aligned with SlainteCare and the National Service Plan (NSP). It is also aligned to principles set out in the National Framework for the Integrated Prevention and Management of Chronic Disease in Ireland (2020\-2025\) and The HSE National Strategic Plan to improve the health of people experiencing homelessness in Ireland (2024\-2027\).

• Provide a specialist nursing service for patients that requires support and treatment through the continuum of care.


• Undertake a comprehensive patient assessment, including physical psychological, social and spiritual elements of care utilising validated assessment tools.


• Identify the services required for patients ensuring diagnostics and rehabilitation are achieved promptly.


• Support the delivery of sub speciality clinics/ in conjunction with the Advanced Nurse Practitioner /Consultant.


• Liaise with other specialist teams ensuring that the most appropriate care is provided to patients.


• Participate in case review with MDT colleagues.


• Take a proactive role in the formulation and provision of evidence based PPPGs related to the area of specialist practice


• Communicate, negotiate and represent patient's values and decisions in relation to their condition in collaboration with MDT colleagues.


• Develop and support the concept of advocacy particularly in relation to patient participation in decision making thereby enabling informed choice of treatment options


• Provide relevant education, in terms of knowledge, skills and attitudes for all healthcare staff.


• Maintain a record of clinically relevant data aligned to national KPI's as directed and advised by DON and senior management

• Be a registered nurse/midwife on the active Register of Nurses and Midwives held by An Bord Altranais and Cnaimhseachais na hEireann (NMBI / Nursing and Midwifery Board of Ireland) or be eligible to be so registered.


• Be registered in the division(s) of the NMBI Register for which the application is being made or be entitled to be so registered.


• Have a minimum of 1 years' post registration full time experience or an aggregate of 1 years' full time experience in the division of the register in which the application is being made.


• Have a minimum of 1 years' experience or an aggregate of 1 years' full time experience in specialist area of, *OR* if the applicant does not possess the relevant specialist experience, they will be supported to attain one year's clinical specialist experience.


• Have successfully completed a post registration programme of study, as certified by the education provider which verifies that the applicant has achieved a *Quality and Qualifications Ireland* (QQI), *National Framework of Qualifications* (NFQ) major academic Level 9 or higher award that is relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the *National Council for Nursing and Midwifery 4th ed* (2008\). Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery, *OR*


• If the applicant does not possess the relevant QQI NFQ Level 9 qualification, the applicant will be supported to undertake the required postgraduate education, at QQI NFQ level 9 qualification (equivalent to 60 ECTS or above) relevant to the specialist area. This must be achieved within 2 years for this pathway.


• Be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area or will be supported to obtain the required CPD.


• Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice.

• Given the developmental nature of the Candidate role, appointment to this post will be made on a Specific Purpose Contract, directly linked to participation in and completion of the approved candidacy pathway.


• The purpose of the contract is to enable the postholder to undertake the required education, training, and competency development necessary to meet the criteria for progression to the substantive (CNS).


• This wholetime appointment is subject to the terms set out as follows: The post\-holder is expected to demonstrate achievement of the core and specific competencies deemed necessary to manage the identified scope of practice and successfully complete the required educational programme within the agreed pathway timeframe of 2 years.
