Pay rate to £26.05 per hour, 4 days per week

Pertemps are recruiting for an experienced Children’s Emotional Health Triage Referral Co-Ordinator in the Newbury area.
Essential Requirements for this job

  • You will need to have worked in Local Authority Safeguarding, this could be as a Family Support Worker, or in a MASH team. Or maybe you are a Newly Qualified Children’s Social Worker looking to gain paid employment at the end of your course?

  • You will need to be local to West Berkshire – this is not a remote role

  • You will need to be able to start at short notice – this is a a Contract role.

Duties:
  • Log, process and coordinate multiagency information relating to referrals from parents, schools and other professionals.

  • Respond to telephone and email enquiries, provide signposting support and explain the role of multiagency emotional health triage to colleagues, professionals and parents.

  • Build effective working relationships with partner agencies to ensure prompt and effective information exchange.

  • Use initiative to review referrals, provide appropriate signposting information and seek managerial support promptly where information raises concern.

  • With managerial support, screen referrals for serious or urgent risk and escalate to specialist or safeguarding services where required.

  • Provide accurate chronologies, records and analysis of information gathered.

  • Record recommendations for next steps and communicate decisions promptly to the referrer or family.

  • Monitor a child or young person’s outcomes 10–12 weeks after discussion and report back to partners.

  • Attend weekly multiagency triage meetings, complete minutes and create action plans and liaise with parents and professionals to obtain relevant information for meetings and reviews.

  • Assist with issuing action plans to parents or carers, referrers and services providing recommended support.
