This role is ideal for an individual wishing to take the next step in their care career
\*\*FULL TRAINING PROVIDED\*\*
There is scope for this to be a full time role also. Full support will be given by our current team members, the Assistant Care Manager and Registered Manager.
As the company’s Care Assessor, you will be required to visit new clients that wish to receive care from us. Or, potential new clients that would like to hear from multiple providers like ourselves – meaning that representing our company in a professional and caring way matters, to ensure that they chose us as their home care provider.
The role involves assessing the new client for their specific care needs, including: conducting a mobility assessment, capacity assessment, medication assessment, personal care needs, and all other aspects of daily living like cleaning tasks, and hydration/nutritional needs.
From this, you will then be required to write up a full and person\-centred care plan with all relevant risk assessments, before ensuring the care package gets off to a timely and effective start. You will then support with maintaining these care packages by carrying out re\-assessments and reviews with the clients families to ensure that we are providing the best service possible!
