Care Navigator
About the Role Are you a customer service professional looking to start a rewarding career in healthcare?
We are looking for an adaptable and patient\-focused Care Navigator / Medical Receptionist to join our busy General Practice team. No previous medical experience is required — if you have a strong background in customer service and a passion for helping people, we will provide full training for everything else.
You will be the first point of contact for our patients, playing a vital role in ensuring they receive the right care at the right time. This is a varied role where you will actively navigate patient needs, manage prescription workflows, and keep our clinical administration running smoothly.
Working Hours
- 20 hours per week
- Monday to Friday.
- Candidates must be flexible to work shift patterns between 8:00 am and 6:30 pm.
Patient Care \& Navigation
- Act as a Care Navigator, triaging incoming calls to ensure patients are directed to the most appropriate clinician or service.
- Process appointment requests accurately, both over the telephone and face\-to\-face.
- Register new patients and process change\-of\-address requests.
- Prepare home visit documentation for visiting clinicians.
- Proactively call lists of patients to book required appointments and clinics.
- Process repeat prescription requests (via phone, email, slips, and online access) within a strict 48\-hour timeframe.
- Generate requests for non\-repeat medication on behalf of patients.
- Encourage patients to take responsibility for ordering their repeat prescriptions and increase the uptake of the Electronic Prescription Service (EPS).
- Process daily administrative tasks and scan clinical documents onto patient records.
- Ensure all incoming and internal patient correspondence is correctly stored.
- Maintain the clinical computer system securely and assist with gathering statistics when required.
- Comply strictly with Information Governance and confidentiality legislation at all times.
What you need:
- Customer Service Experience: A background in retail, hospitality, administration, or any customer\-facing role.
- Excellent Communication: A confident, empathetic, and professional telephone manner.
- Resilience: The ability to work accurately under pressure in a fast\-paced environment.
- Basic IT Skills: Comfort using computers and learning new software.
- How to safely and effectively Care Navigate and triage patient calls.
- How to use our clinical systems (EMIS Web and DOCMAN).
- How to process medical documents, clinical codes, and prescription workflows.
