Overview

We are seeking a passionate and dedicated Care Manager to lead and coordinate exceptional senior care services within our care home environment. This vital role involves overseeing the delivery of personalized care plans, supervising staff, and ensuring a safe, respectful, and engaging atmosphere for residents. The Care Manager will serve as a compassionate leader committed to enhancing the quality of life for seniors, including those with dementia and Alzheimer’s disease, through expert management and empathetic support. Duties

  • Develop, implement, and regularly review individualized care plans tailored to each resident’s needs, preferences, and medical requirements.

  • Supervise and mentor care staff, ensuring high standards of assisted living, nursing home, and home care services are consistently maintained.

  • Oversee medication administration processes to guarantee safety, accuracy, and compliance with healthcare regulations.

  • Coordinate with healthcare professionals to monitor residents’ health status, manage chronic conditions, and facilitate necessary treatments.

  • Lead efforts in dementia and Alzheimer’s care by employing specialized techniques that promote resident comfort, safety, and engagement.

  • Ensure compliance with all relevant regulations related to senior care, nursing standards, and care home policies.

  • Foster a positive environment by promoting teamwork, providing ongoing training opportunities, and encouraging open communication among staff members.
Skills
  • Proven experience in assisted living or nursing home settings with a strong understanding of senior care practices.

  • Demonstrated leadership and supervising experience within a care home or similar environment.

  • In\-depth knowledge of dementia care strategies and Alzheimer’s disease management techniques.

  • Expertise in developing comprehensive care plans that address physical health, mental well\-being, and emotional support.

  • Strong management skills with the ability to coordinate multidisciplinary teams effectively.

  • Excellent communication skills to engage residents’ families, healthcare providers, and team members transparently.

  • Familiarity with medication administration protocols and regulatory compliance standards related to nursing and home care services.
