We are currently looking for a highly organized, compassionate, and proactive Care Coordinator to join our fast\-paced Wolverhmpton office. This is a pivotal role where your expertise will directly impact the lives of our service users. You will act as the beating heart of our operations, ensuring that the right care is delivered at the right time, by the right person.
What You’ll Be Doing:
- Rota Management: Orchestrating complex scheduling to ensure all care visits are fully covered, minimizing gaps, and managing last\-minute changes with efficiency.
- The Go\-To Liaison: Serving as the primary point of contact for service users, their families, and multidisciplinary healthcare professionals to ensure seamless communication.
- Person\-Centered Planning: Proactively managing cases by developing and reviewing Health Action Plans and monitoring progress toward patient goals.
- Team Leadership: Taking the lead on recruiting, vetting, and onboarding new support staff, as well as overseeing their ongoing professional development.
- Operational Excellence: Handling administrative tasks including accurate invoicing and ensuring the company remains in strict compliance with regulatory standards (CQC).
- Experience: You have prior experience in a healthcare or domiciliary care setting.
- Tech\-Savvy: You are proficient in electronic care management software and comfortable managing digital administrative systems.
- Multitasker: You have an exceptional ability to juggle multiple priorities without compromising on attention to detail.
- Communication Pro: You are professional, empathetic, and capable of building strong relationships with staff and service users alike.
- Problem Solver: You remain calm in a fast\-paced environment and have a "can\-do" attitude toward scheduling and staffing conflicts.
