Overview
Prime Way Care Ltd is looking for a reliable and organised Care Coordinator to join our team. You will support clients, families, and care staff to ensure safe, high\-quality, person\-centred care is delivered.
This role involves coordinating care services, supporting communication, keeping accurate records, and helping resolve day\-to\-day care issues.
Key Responsibilities
- Coordinate care services and daily schedules
- Act as the main point of contact for clients, families, and professionals
- Monitor client needs and update care plans as required
- Support problem\-solving and address care\-related concerns
- Maintain accurate records and documentation
- Ensure compliance with company policies and care standards
- Strong organisational and communication skills
- Ability to manage multiple tasks
- Full UK driving licence/preferred
- Flexible, reliable, and team\-focused
- Basic IT skills (Microsoft Office)
- Relevant degree advantageous but not essential
- Ability to speak Somali is desirable
