Care Coordinator (Domiciliary Care)

Location: Gloucestershire
Salary: £26,000 – £30,000 per annum (depending on experience)
Contract: Full\-time \| Permanent
Start Date: ASAP

Role Type \& Hours

  • Full\-time

  • Monday to Friday (flexibility required)

  • Participation in the on\-call rota as required
Please note: This is a community\-based role. Applicants must hold a full UK driving licence and have access to their own vehicle. Travel to service users’ homes, staff spot checks, and reviews is an essential part of the role.

Job Summary

We are seeking an experienced and organised Care Coordinator to support the effective coordination and delivery of our domiciliary care services. This role plays a vital part in ensuring service users receive safe, high\-quality care while supporting care staff through effective rota management, communication, and quality oversight.

This is a community\-based role and requires regular travel to service users’ homes for reviews, spot checks, and quality monitoring.

Key Responsibilities

Care Coordination \& Service Delivery

  • Coordinate care packages in line with care plans and assessed needs

  • Ensure continuity of care and appropriate allocation of care staff

  • Respond promptly to changes, emergencies, or staffing issues

  • Conduct service user reviews and support care plan updates

  • Provide direct care in emergencies where required
Rota \& Staff Management
  • Create and manage staff rotas using electronic care systems

  • Allocate care calls efficiently, considering skills, availability, and travel

  • Act as first point of contact for care staff during shifts

  • Provide guidance, advice, coaching, and problem\-solving support
Quality, Safeguarding \& Compliance
  • Carry out spot checks and quality monitoring visits

  • Identify, report, and respond to safeguarding concerns

  • Support audits, investigations, and CQC inspection readiness

  • Ensure care delivery meets CQC Fundamental Standards
Communication \& Administration
  • Maintain regular communication with service users and families

  • Liaise with local authorities, healthcare professionals, and commissioners

  • Maintain accurate care records, schedules, and documentation

  • Support onboarding of new service users and staff
Skills, Knowledge \& Experience

Essential

  • Experience working within domiciliary care or health \& social care

  • Strong organisational and time\-management skills

  • Excellent communication and interpersonal skills

  • Good IT skills and experience using care management/rota systems

  • Sound understanding of safeguarding, CQC standards, and person\-centred care

  • Full UK driving licence and access to own vehicle
Desirable
  • NVQ/QCF Level 3 (or above) in Health \& Social Care (or working towards)

  • Previous experience as a Care Coordinator or Senior Carer

  • Knowledge of local authority commissioning and care packages
