Job Overview

We are seeking a proactive and compassionate Care Coordinator to join our domiciliary care team. In this vital role, you will oversee the delivery of high\-quality home care services, ensuring each client receives personalized support that promotes independence and well\-being. Your leadership will help coordinate care plans, supervise care staff, and foster strong communication with clients, families, and healthcare professionals. Responsibilities

  • Develop and review tailored care plans to meet individual client needs, ensuring compliance with regulatory standards.

  • Coordinate schedules for care staff, ensuring timely and efficient service delivery across all client visits.

  • Complete care calls when necessary to maintain safe staffing and continuity of care

  • Supervise and support care staff, providing guidance, training, and performance feedback to maintain high standards of care.

  • Communicate effectively with clients, families, healthcare teams, and external agencies to ensure seamless care coordination.

  • Conduct regular assessments of clients’ conditions and update care plans accordingly to reflect changing needs.

  • Monitor service quality through spot checks and feedback collection, addressing any issues promptly.

  • Maintain accurate documentation using our care management system for scheduling, care plans, and incident reports to ensure transparency and compliance.

  • Ensure compliance with company policies and regularoty standards.

  • Promote safeguarding awareness and report concerns immeidately in accordance with company policy

  • Support the appropriate reporting and review of incidents, accidents, medication concerns, complaints and near misses.

  • Assist with audits, quality monitoring and completion of improvement actions.

  • Handle confidential and sensitive information in accordance with UK GDPR, data protection requirements and company policy.

  • Participate in governance meetings and contribute to lessons learned and service improvement.

  • Work in accordance with CQC regulations, quality statements, the Health and social care Act 2008 and Robethel Support Service Ltd Policies and procedures.
Skills
  • Strong driving skills with a valid licence to facilitate client visits and staff transportation \- own car for work

  • Minimum of 2\-3 years' experience working in domiciliary care settings or senior care environments with supervising responsibilities.

  • Proven leadership skills with the ability to motivate and guide a team of care professionals.

  • Excellent communication skills to liaise effectively with clients, families, and multidisciplinary teams.

  • Ability to develop comprehensive care plans tailored to individual needs while adhering to regulatory standards.

  • Proficiency in IT systems for scheduling, documentation, and reporting purposes.

  • Supervising experience in a healthcare setting is highly desirable for success in this role.

  • Ability to work under pressure and solve problems effectively

  • Knowledge of care regulations and safeguarding procedures
