Summary
We are seeking a Care Co\-ordinator/ Care Supervisor to join our team in Brighton. Based in the local area, this role is vital in ensuring our clients receive tailored support that promotes their independence and well\-being.
Job Responsibilities
- To provide efficient customer service by maintaining effective links and communication with local authority, care workers, service users, relatives and any other relevant individuals
- Ensure weekly rotas are completed in advance, ready to publish a week in advance.
- Support the Registered Manager to manage the service and staff in accordance with the service’s aims and objectives, policies and procedures.
- Assist with full Care Plan assessments.
- Complete all assessment paperwork as per CQC standers.
- Complete service users year reviews/surveys.
- Complete Carers supervisions,spot checks as required as per CQC standers
- Ensure Audits and all documents are up to date as per CQC standers
- To record and report any complaints while ensuring that all quality standards, complaints and comments are dealt with promptly.
- Great communication skill on and off the phone.
- To manage the Administrative Assistant to ensure efficient general clerical and administrative support for the White Heart Care.
- To implement and monitor all care packages and care plans taking appropriate and remedial action where necessary, in consultation with Registered Manager
- To ensure all care workers are appropriately trained and able to carry out their duties and responsibilities effectively and sensitively providing person centered approaches.
- On call duties as per Rota basis, on call paid in addition to your salary.
- To manage the care workers rotas and workloads, ensuring that Health and Safety, Working Time regulations are considered.
- In collaboration with the Registered Manager to deal with emergencies arising from the service
- To administer the care workers recruitment process ensuring that all care workers are fully compliant with White Heart Care’s recruitment and induction procedures
- To regularly review care workers training needs to ensure that any developmental training needs are met
- To liaise with the Finance and Payroll teams and other internal White Heart Care Services as required.
- To ensure accurate records are kept and maintained through using the organisations database \& case management systems in accordance with the GDPR.
- To produce reports and statistics as required.
- To assist in the induction and ongoing support of volunteers in line with the investing in Volunteers Code of Practice.
- Great Time Management.
- Ensure that White Heart Care’s policies and procedures are adhered to, particularly those relating to Equal Opportunities, Confidentiality and Health \& Safety.
- To perform other tasks when requested which are not detailed above but are essential in the smooth running of the services, as directed by the Registered Manager.
